Withdrawal Form

Please discontinue automatic payments to my credit card on file. I understand that my child’s spot in that class is no longer held once I withdraw. Students may attend special activities without being enrolled in a class and may return to the class program at any time. Registration fees are good for one year from the date of payment.

Withdrawal must be received by the 27th of the previous month in order to be withdrawn.

(Ex. If withdrawing for the month of October, the notice must be received by September 20th.)

Notices received after the 27th will result in a charge for the following month and will push back the withdrawal date to the following month.

Please note: You are responsible for payment for your student’s classes WHETHER OR NOT YOUR STUDENT ATTENDS CLASS until the time you notify Premier Gymnastics VIA THIS WITHDRAWAL FORM NOTICE. 

Please do not rely on your student to verbally let us know that he/she will no longer be attending classes.  NO REFUNDS WILL BE GIVEN!

*All competitive team athletes must contact their coach prior to withdrawal.
